Create your child's profile and action plan
After you have a parent / adult account, create a profile for your child. The profile holds their basic information and the allergy action plan you (and later a physician, if you ask for review) complete in AllergyAIDE.
Start a new profile
Section titled “Start a new profile”- Sign in to AllergyAIDE.
- Choose to add a profile (or follow the prompts after a school/camp YES or Create Action Plan link).
- Enter your child’s details as the form asks (name, date of birth, and related fields).
- Complete the allergy and action plan sections — allergens, medications/devices, emergency contacts, and any consent steps shown on screen.
- Save / submit when the form is complete.
If you opened a school or camp request link, AllergyAIDE connects this profile to that request so you can share the finished plan back later.
Tips while filling the form
Section titled “Tips while filling the form”- Have your child’s epinephrine device information and emergency contacts nearby.
- Answer only what the form asks — you can edit the profile later from the child’s profile page.
- If you arrived from a school or camp request, finish this profile for that child before starting another.
After the profile is created
Section titled “After the profile is created”You land on the child’s profile. The action plan timeline shows your progress.
- School or daycare request: Ask a physician to review is required, then sign and share.
- Camp request: physician review is recommended, or use Proceed without review, then sign and share.
- No institution request: physician review is optional — see Create a plan on your own.
